Who is responsible
CloudHospital Inc. is responsible for personal information submitted through the K-Med Beauty consultation form. K-Med Beauty is the customer-facing service used to collect the consultation request.
Information collected
The required information is your current situation, relevant Korea area, consultation category, timing, name, home country or region, email address, reply language and preferred contact method. A phone number or messenger ID is required only when phone, WhatsApp, WeChat or LINE is selected. The page path, locale, source context and selected clinic context may be attached to the request.
Why it is used
The information is used to receive the request, understand the practical context, reply in the selected language and channel, and prepare availability, quote, booking, interpretation or coordination questions when requested. It is not used to make a medical diagnosis or treatment decision.
Processing and storage
The validated request is processed and stored using access-controlled Google services used for K-Med Beauty intake. Internal notification is designed not to include the lead’s personal information.
Retention
Consultation intake information is retained for 90 days and then deleted under the current intake policy.
Clinic sharing
The request is not disclosed to a clinic or another external recipient through this form. If that disclosure becomes useful, K-Med Beauty must first show the recipient and purpose and obtain a separate consent.
Your choice and requests
You may choose not to agree, but the consultation request cannot be submitted without the required intake consent. You may request access, correction or deletion by using the K-Med Beauty reply channel associated with your request.
Sensitive information and analytics
Do not submit medical photos, records, passport details, resident identification numbers or other sensitive medical information. Optional analytics consent is separate from consultation consent and can be declined without blocking the consultation form.
